Please attach the output of the printingbug info script from the server and
your /etc/cups/cupsd.conf, the printingbug info script can be found here:

Also attach your /var/log/cups/access_log and your cups error_log as described
here: https://wiki.ubuntu.com/DebuggingPrintingProblems#cupserrorlog, after
trying to add the printer from a Windows machine.
