update-grub only updates/adds the kernels between "### BEGIN AUTOMAGIC KERNELS LIST" and "### END DEBIAN AUTOMAGIC KERNELS LIST". If you don't have this section it won't do anything. If you modify manually what's between these two markers (which you are not supposed to do in normal cases) it will detect this and ask through the ucf prompt. At least that's how I understand it should work. Can you please attach your menu.lst so that we can see what your case is?
