It appears when one asks for bigger icons in the kontact/kmail toolbars. It seems to come from incomplete icon themes, for instance the icon named "mail_todo.png". For CrystalSVG theme, a kmail crystalsvg iconset is in the folder /usr/share/apps/kmail/icons/.
