My initial assessment was correct.

Referring to http://bugzilla.gnome.org/show_bug.cgi?id=343437 it would
appear that the panel sometimes chooses the wrong size for the icon if
both 22x22 and 24x24 icons are installed.

hicolor-icon-theme has been patched by seb128 so that
01_dont_scale_22x22_apps_icons_for_hicolor.patch is applied.

This fixes the menu, but breaks the notification area and panel.

The really-correct fix is to revert this patch and apply the patch
mentioned in #343437 to gnome-panel. I have done this on my machine, and
both the notification area and the panel are now correct (icons are
crisp and sharp), although this does make the default menu item size
22x22 and makes some of the icons without 22x22 versions look a bit
cloudy.

Up to you guys what you want to do.
