So after the laptop woke up from hibernation with the sound not working, I did 
two experiments.
First, I plugged in some USB speakers and they worked just fine.  

Secondly I shutdown all /dev/dsp using software and slowly removed
kernel modules until i could remove snd_hda_intel and then reload it

And after that sound worked again.  So this really seems to point to
some small reinitialize hardware bug on wake up in the snd_hda_intel
kernel module.

Also, this bug (no sound) occurs when the laptop wakes up from a
suspend.
