I can confirm this bug, although it is somewhat hard to reproduce on my setup. It's a bit of a hit and miss affair whether the menu entry ends up in lost and found or not, and I have not been able to find or see a common denominator. When I edited the entry for sweeper, it ended up in lost and found but it didn't for Okular (although Okular did lose its icon). Editing juk (i.e. moving it to a new submenu of multimedia) worked as it should.
