I've used Dragon Naturally Speaking and IBM's Via Voice. Both are closed source, but would make great open source projects if they are ever opened up. IBM uses Via Voice in many automated telephone answering systems, so I doubt that would be opened anytime soon.
This seems like a good Google summer-of-code project.
