There is something with that layout. With that enabled I can reproduce the problem. Both on jammy and impish. >> 2. Unlike when using for instance the Swedish or "English (US, intl., with dead keys)" layout, 'compose:ralt' does not override the layout's default definition of "Right Alt" as the 3rd level key. Can it be some kind of race condition? So it seems to be the combination of Right Alt as compose key and the English (Dvorak, alt. intl.) layout which does not work. I changed the bug title accordingly. And AFAICT gnome-control-center sets the expected XKB options, so it's not a g-c-c bug.
