I applied this fix to `~/.config/pulse/default.pa` > rebooted:
```
.include /etc/pulse/default.pa
unload-module module-suspend-on-idle
```

Seems to have fixed it - maybe dialogues were waiting for pulse audio to
wake-up to play the alert sound.
