I think I figured out what's happening. xfwm4 is still adding the Ctrl + Alt + D shortcut for new users. Try this and let me know if it works.
1. Apply the attached diff to /etc/xdg/xdg-xubuntu/xfce4/xfconf/xfce-perchannel-xml/xfce4-keyboard-shortcuts.xml 2. Create a new user (Menu > Users and Groups) 3. Login as the new user 4.
