Public bug reported:

In USN-4227-1 a new kernel (4.15.0-74) implementing a change in the QLogic 
Fibre Channel driver was introduced.
With the new kernel, the connected Tape drives/changers (2 changers, 1 drive) 
are not found.
Repeatedly restarting the subsystem resulted in either
- No tape drive found
- 1 tape drive found
- 2 tape drives found
- But NEVER all 3 tape drives found

Reverting back to kernel 4.15.0-72 resolved the problem.
All newer kernel packages also have that problem.

I attached both the outputs from kernel 4.15.0-72 (working) and kernel
4.15.0-88 (not working) in TGZ Format
