Interesting ordering in #1. It seems the "Failed to sync" lets an I/O hang which is then later reported failed. But you could unmount & unbind the device cleanly on the host before you hot-add it to the guest. Then for case #1 none of the errors should appear I'd hope. Do you want to give that a try Erro?
