Public bug reported:

Going through the different options, not changing anything, the buttons in the 
title bar randomly disappear.
Maximizing (for example) get them back on.

Found that message in the syslog, at the time of the problem appearing :
gnome-tweaks[16316]: Negative content width -23 (allocation 1, extents 12x12) 
while allocating gadget (node label, owner GtkLabel)

That happens on an ubuntu fresh install, just added gnome-tweak-tool.
