I have a similar set up and issue.  
I've plugged in my headphones and the "dongle" into my headhpone and mic jacks 
on my sound blaster Z.
No sound when I first install Kubuntu.  I default to using headphones.

I disabled the onboard creative audio in the BIOS when I first purchased
the system with the sound blaster Z.  The onboard audio does not show up
in Kubuntu (expected).

The Nvidia HDMI sound ports are both seen and take priority.  I cannot
change the priority order in the volume control gui.

Also, there are no other mixer options except for a very basic slider in
the volume control gui (Pulse Audio).

If I run the alsamixer app from the command line and unmute the
headphones I can head sound.  In order to save my configs I have to use
the alsactl store command.

All of this is an inconvenience and not the end of the world.  That
said, it would be nice to have more volume sliders in the Pulse Audio
GUI and change the priority of the devices so that HDMI is not seen as
the primary device.
