Public bug reported:

An installation broke some pulseaudio files. Ubuntu was enable to
restart, coming back to welcome screen as requiring new login. Seemed an
endless loop. Booted to Windows and prepared USB Stick with iso image.
Everything works until Ubuntu installation says there is no UEFI
partition. The one that is shown has the Windows Boot Manager. Button to
add partition ( + ) is inactive. All seems to have begun after trying to
install icons distributed by OCS - Open Collaborations Systems.
