I think we'll also need to see what's in /boot/efi in general:

sudo ls -latrR /boot/efi

There is absolutely no reason for mmx64.efi to be missing, ever -- it's
the indication of an incomplete install, or at the very least an issue
with the firmware of the system. We always install mmx64.efi.
