Hi, It is maybe a hardware failure[1]. Does this bug is still present with more recent exfat-fuse or in a more recent Ubuntu (at least in Xenial)? Last versions of fuse-exfat fixes many bugs. If it is still present, can you report this issue to the upstream developer[2]? He will be able to confirm that it is a bug in exfat-fuse or a hardware failure.
