This is not how the preseeds work with d-i on Debian/Ubuntu.
Specifying s390-zfcp/zfcp like that preseeds the answers for that question,
however the question may still be shown.
The questions shown depend on the priority of the installer (default is high)
and thus this question will still be asked.
If you are trying to achieve fully automated installation, you would
need to raise the priority of the installer to critical only; but also
preseed answers for many other questions too.
If you can still reproduce the problem, please extract and attach
installer logs /var/log/syslog from the running / crashed installer.
