Bug #1636009 expired but this here is a more detailed and targeted
analysis.

Recent experiments demonstrate that the NVidia card does not get initialized on 
reboot at all.
If powered of and turned on, it seems that the NVidia card keeps some 
information for some time thus reinitialization does not take place.

After reinstalling the driver (after purging), the driver does not
initialize the card any more.

Reverting to a bare metal backup of 16.04 demonstrates that the card
a) boots normally
b) serves HDMI ports on both thunderbolt ports and the primary HDMI port

Thus: I attribute it to the driver
