added the following to /etc/modprobe.d/alsa.conf
options snd-hda-intel single_cmd=1
options snd-hda-intel probe_mask=1
options snd-hda-intel model=generic

Now I only ever see one output device; speakers or headphones (when
plugged in). HDMI doesn't recognise at all (although that's not a major
issue since it doesn't output video anyway)

Setup;
PC: XPS-9360
OS: Ubuntu-16.04
BIOS: 1.0.7
Kernel: 4.4.0-53-generic
