So far Calibre's Ebook viewer and Encryptr gave me this kind of problem.

to reproduce: open any of those 2 applications.

What happens is that the application's icon is not properly displayed in
the favorites, then the icons for other open non-favorite applications
do not disappear when closed, or disappear after a very long time, and
icons of newly open applications fail to appear.
