Discovered it also happened on the primary screen, and this time
randomly.

** Summary changed:

- volume control sometimes incorrect on second display
+ volume control status sometimes incorrect

** Description changed:

Occasionally, when using the media keys with the mouse on the second
display, the volume control that appears will show 0 volume. Pressing
the media keys again reveals the volume I expect (70-90%). The volume
is actually at a good level when this happens, so it is strictly a
  status display problem.

I believe this happens when using the media (volume adjust) keys on the
second screen for the first time. I have not noticed it happening on my
primary screen. I use the nvidia driver w/ 2 displays combined to a
  2960x1050 desktop.
