Hi Phillip, After 20+ attempts with various configurations I gave up and left /dev/sda out of the raid array (i.e., /dev/sda as / and /dev/md0 as /home). I have just finished my reinstall and so I'm afraid I no longer have access to the requested logs. Maybe somebody else experiencing the same issue in the future will be able to provide the additional info.
A quick aside, even after removing /dev/sda from the raid array and using manual partitioning the system the installation was far from smooth. The installer correctly added # /home was on /dev/md0 during installation UUID=fe8c429c-1700-4210-aa76-3dd8d5fd91aa /home ext4 defaults 0 2 to /etc/fstab but didn't install the mdadm package. As a result the system hung on reboot.
