I also had this problem on a recently upgraded Toshiba A100 series
laptop.  All worked fine on Dapper.  I upgraded through Edgy to Feisty
(2.6.20) in the last couple of days and had this USB mouse problem.

-- USB mouse went dead at seemingly random times

-- Unplugging/replugging did NOT fix it for me

-- Deactivating USB legacy device support in the BIOS did NOT fix it for
me

-- modprobe steps mentioned above hung for me, too

-- Adding "acpi=force irqpoll" as boot options in GRUB looks like it DID
cure it (so far, so good)
