The "ips" driver is for the IBM ServeRAID. The dapper install CD used driver version 7.12.02, and gutsy-devel is using the same version. On the dapper boot CD, casper/initrd.gz contains the ips driver.
Possible causes might be (1) a new revision of the ServeRAID chip that isn't listed in pcitable in dapper, (2) the CD isn't trying to load the driver when looking for the CD, (3) bad CD, or (4) bad CD drive. Please provide the following information to help narrow this down: (1) Is this still happening in current versions of ubuntu? (2) If it's still happening, try running "Check CD for defects" when booting the install CD. That should rule out faulty media and faulty CD drive.
