I'm also having this issue on 12.04 and 14.04. For me, adding the manual 0000 pin solves the problem after power off/on but does NOT solve the problem after channel switch, where a manual reconnect is required.
Also, on both 12.04 and 14.04, I'm having occasional issues where I'm getting a keypress continuously (a regular key on the keyboard like a letter or a punctuation mark). I'm suspecting a gesture message somehow being misinterpreted by X, but I'm not able to reproduce it reliably. It's really annoying and it's fixed by turning the mouse on and off, which causes it to reconnect.
