I can confirm same problem and solution - missing device id's and USB3 
incompatibility (very sporadic working) for HP ProBook 450 G1.
Kernel id patch and custom compiled kernel with disabled USB3 completely 
resolves bluetooth problems.

Custom kernel is same as original kernel, except disabled USB3 and patched 
ath3k.ko and btusb.ko, compiled from https://github.com/Satius/mod_0cf3_311f 
source.

Unfortunately from tomorrow i'll be mostly unable to test any changes,
since that computer goes away in production.
