Same here, Ubuntu 12.04 LTS unity 5.18.0 I have 5 launcher on my bar: chrome, firefox, sublime text, terminal, and a custom launcher that launches a bash script thant launch the 4 icon mentioned before. This behavoiur does not happen always in the same form.
Sometimes the icon duplicated is the chrome one, sometimes the terminal, and sometimes is firefox. When I icon is duplicated, if i click on the launcher icon instead of giving me the focus on the opened window it create another terminal/firefox window/chrome window.
