It's not proving to be easy to reproduce this atm on my 965gm (which is slightly older than your gm45 but a very close relative). As demonstrated earlier, the bug is likely to be further internal state of the GPU becoming corrupt - and probably the VUEs again as we have numerous examples of that being broken in gen4 GPUs.
