Most upstream names for Qt creator plugins seem to follow the naming scheme: qtcreator-plugin-$NAME I think it would be better to stick with this and use qtcreator-plugin-ubuntu for the name of the new binary. Also, you'll need to find an archive admin with time to do the New processing for this. Pre-Depends: dpkg (>= 1.15.6) is not needed on Ubuntu. I don't think the botan copyright is needed, since we patch it to build against the system botan and not the embedded copy. Also, you'll need to add a full copy of GPL v3 to the source tarball. While referring to /usr/share/common-licenses is fine for debian/copyright, the tarball needs to be distributable outside of a Debian/Debian based system, so the full license is needed. Also, it looks like Canonical's choice of license will be a barrier to eventual upstreaming: http://qt-project.org/legal.html - you might want to start working now.
