The log shows that you have a locally-installed 2.6.39.4 kernel image in /boot, but you do not have a corresponding /lib/modules directory for this kernel version. This is not a bug in initramfs-tools, but a bug with the set of kernels you have present on the system. You should remove /boot/vmlinuz-2.6.39.4, since it clearly shouldn't be there.
