I've got exactly the same issue. I'm using Fiesty and OO 2.2, the button "Use Wizard to Create Form" does nothing when clicked. I remember having a similar issue to the comment above when I used Edgy, I was able to design the form using the wizard, but the finish button didn't work so I couldn't save it. This is very frustrating, I'm hoping to train someone else who needs to be able to do this.
