Ran some more tests.  After noting the BIOS setup correlation, I
replaced the BIOS battery and reflashed the the BIOS.  This did not
appear to have any effect.

I just tried creating a modprobe conf file with "alias eth0 e1000e".
After a reboot the NIC was back and functioning.
