Using a fresh install of 12.04 on a 2012 model Sony Vaio SVS15116GG, I had this same problem - my Logitech MX Anywhere mouse stopped working sometimes. Occurs after suspend, on fresh boot, etc. It seems fairly unpredictable what causes it, but sometimes even booting fresh without the receiver plugged in, and then plugging in (or unplugging/replugging failed). I was just caught in the situation with unplug/replug not helping, and there seemed to be no way to get it working again. The suggestion from an earlier commenter to try unplug/replug after locking the screen worked!
