ok, this happened to me: I was installed couchdb-bin package instead of
couchdb.

With only couchdb-bin package,  I don't have the /etc/init.d/couchdb.
Installing and following your instructions I dont have any problem.

Could be feasible add a simple user detection for permission at begging
of /usr/bin/couchdb?

Something like:

$ /usr/bin/couchdb
Aborting. Please don't run couchdb binary as regular user. Use instead sudo 
/etc/init.d/couchdb start

This message could be more useful for begginers that crash dump and save
some invalid duplicates.
