This bug has shown up on 12.04 again. My system started up as described above with the exception that I can click on the menu bar and close a program by clicking on File -> close - just no title bar or window decorations. In addition, the focus was not properly directed to open windows and I could not type in my password in Keepass, for example, but I could click on the cancel button. The top panel showed open windows that I had previously closed. I was able to restore the window manager by typing xfwm4 --replace in a terminal window. I can attach additional logs if that would be helpful, just not sure which ones.
