I had this message spammed in my logs on a Dell R515, and thought it was
a bug. However, it turns out it really was a hardware fault. Dell's
built in 'system services' has a self-test, and running this self-test
reliably reported a SAS-related failure code. Replacing the SAS card +
backplane solved the issues.
