Public bug reported:

When running dual monitors, the left screen toolbar only allows new
shortcuts to be drag and dropped.

On right screen monitor, click dash, search terminal, drag terminal to unity 
bar until "drop to add application: and nothing happens.
Perform same action on left screen, it adds the shortcut to unity bar.

Unity is activing differently on right primary screen. Using nvidia
primary right screen with nvidia-settings.
