I've seen that toggling mute with ALSA (amixer) can leave PulseAudio in a muted state and the only way to unmute it is to run 'pactl set-sink- mute $CARDS 1' where $CARDS is the index of your audio sink (you can try to replace it with 0 or 1). I've modified a simple script that you can bind to the XF86AudioMute key to toggle mute. It will toggle mute for all the sinks present which usually isn't an issue. The directions for the key binding in the script are for XFCE (I use Xubuntu) so yours may be in another location. Let me know if this helps.
