I'm not sure if this is a Unity or a compiz bug. It is similar to the
reported problem in
https://bugs.launchpad.net/ubuntu/+source/compiz/+bug/875400 and

http://ubuntuforums.org/showthread.php?t=1861738&highlight=compiz
After what seems like any change in ccsm I loose the launch bar, dash
and top menu bar. I'm going to try to mark this as a duplicate of
875400 but I wanted the apport additional information attached and can't
seem to find the proper description to mark it as such here.
