Hasn't been fixed for a year yet. I had to manually update both revtex4-1 to its latest version to get affiliations to work properly AND natbib to compile a paper. A LTS version should not have this problems for so long. Having to manually update latex packages is quite inefficient, one chooses a LTS version for a reason.
