The icon being used is 48x48, and is probably one of the following (I tried digging through the Banshee source code, but couldn't figure it out):
in /usr/share/icons/gnome/48x48/ audio-x-generic.png sound.png gnome-mime-audio.png gnome-mime-application-ogg.png They're all part of the default GNOME icon theme, although Clearlooks seems to be the only theme that causes it. If for example I use the Ambiance theme but customize it to use the GNOME icon set, the icon shows up correctly aligned.
