I'd like to be able to debug this problem better, but I'll need someone to give me directions on how to do that.
* When I use i915.modeset=0 I'm not even sure that the i915 driver is used. Judging by the X log, it looks like a standard VESA driver is being used, with the wrong display resolution. * If I allow the system to boot up normally (to the flickering built in display) vga_switcheroo is available, however when using i915.modeset=0 the vga-switcheroo option is no longer available. * Booting up normally with an external monitor sometimes results in the KDE desktop showing up on the external monitor while the built in display flashes. I can't always recreate this though. Is this a case of the i915 driver not being able to function properly only with the built in display?
