Since you run a Windows host you may wish to follow this discussion:

http://forums.virtualbox.org/viewtopic.php?f=1&t=40922&start=45#p184602

You may find out there, that GA 4.0.4-71416 was ment to be used for windows
hosts. Then this was ported to 4.0.6
This is history now, hence, a newer Version 4.08 is available since 2001-04-16

Besides - you got stuck with a wrong inclusion (I got messed up with this
thinking too, a while ago.)

There is actually no need to to install guest-additions from within
Ubuntu, if it is run as guest.. The Guest system gets established from
the Host-System (this is in your case Windows) supplied with the
package you downloaded from VirtualBox.org

Besides, the Guestadditions have to match the main package, if you try
anything else you may ruin your .vdi

 This bug can be marked as "wont fix".

Best regards

P.S.: VirtualBox supplies one of the best documentions I've seen.
