I confirm this issue. For clarity, the issue is that when launching e.g. a text document, it gets a separate icon (a plain white document), rather than being grouped with the "standard" blue LibreOffice icon.
The separate icon can be closed independently using the right- click/close option of the launcher. The OOo documents associated with the standard button remain open. Reversely also, closing the documents associated with the standard button does not close the separate icon. It is not very consistently reproducible. In the attached screenshot, three documents were grouped with the standard icon, a fourth document got its own icon. I have pretty much the impression that it is related with the time there is needed to open a document. Larger documents that need time to open, are all grouped in the separate launcher. Small documents appear correctly in the blue Writer icon. Attachment illustrates the issue (again) so that it is very clear what we are talking about.
