There seems to be some sort of hardware problem or race condition
because this problem only seems to happen when I turn the computer on
from cold, no matter which kernel version I use. If I do a warm reboot
then it almost never happens.

I've never known the problem to go away without a reboot once it has
manifested itself, nor for that matter to manifest itself without a
reboot if it hasn't at the start.

It's not causing me serious issues any more because I just check it
works when I first boot up and, if not, reboot again and 9 times out
of 10 it fixes it, but it's still a little annoying, and my curiosity
would like to get to the bottom of what's causing it!
