There seems to be some sort of hardware problem or race condition because this problem only seems to happen when I turn the computer on from cold, no matter which kernel version I use. If I do a warm reboot then it almost never happens. I've never known the problem to go away without a reboot once it has manifested itself, nor for that matter to manifest itself without a reboot if it hasn't at the start.
