Ok so after disabling Pulseaudio and switching to ALSA using this guide:

After a couple extra tweaks sound is working pretty much perfect, the
bug is gone. I think this confirms that the bug is in PulseAudio and
since no other distro's are effected it's something Ubuntu specifically
does to PulseAudio. Anyways I'm happy, still want to find a fix but at
least it's something.
