I have some more information on what possibly causes this (or atleast adds to the issue).
The server has a nice RAID array of 2TB discs attached and it's running rtorrent. I've historically had issues with rtorrent fragmenting filesystems so I created a separated 32GiB file and mounted it through loopback as the download location for rtorrent. rtorrent is setup to move files from the download "drive" to the final location once finished, the errors appear to occur when this move happends.
