You might have a hardware problem as is evidenced by this line in dmesg: 'wifi%d: unable to attach hardware: 'Hardware self-test failed' (HAL status 14)'. The other flaky symptoms appear to support this hypothesis. Also, other Thinkpad users seem to be able to use their Atheros adapters without problems.
