Thanks for getting back to me. SO it appears that something in the driver is dying when attempting to deal with your card. Could you plesae boot into a live CD again, and get me the complete dmesg output? To do this, open a terminal, and dump dmesg to a file like so: "dmesg > dmesg.log". Then attach that log file to this bug.
